Payment is required at the time of your appointment.

Please use the permit tester's name in all forms below.

Please have exact change, blue card, your birth certificate, AND YOUR SOCIAL SECURITY NUMBER CARD ON HAND.

*Permit Tests in Princeton, Chisago and North Branch are ONLY held on the last day of in person class. If you’re looking for something immediate, check our main location - Forest Lake. We offer testing daily. Please keep in mind we do NOT book out months in advance.
